
Shaunak Parikh, MS, RN, PMHNP-BC, is the Owner and Founder of Parikh Mental Health Services, PLLC, dedicated to providing exceptional mental health care. He is an ANCC Board Certified Psychiatric and Mental Health Nurse Practitioner, with over a decade of experience providing compassionate evidence-based support for individual struggling with conditions such as depression, anxiety, PTSD, ADHD, Bipolar disorder, Schizophrenia and various other mental health Conditions. Mr. Parikh has honed his skills by providing care and practicing across various settings, including inpatient and outpatient care, ensuring high-quality treatment for his patients. As a strong advocate for Telehealth, he brings over 8 years of experience in delivering accessible mental health support through Virtual care. Mr Parikh provides care and mental health services to individuals above the age of 18 and over. Mr Parikh has 3 BS Degrees in Chemistry, Psychology and Nursing, respectively. Mr Parikh received his BS degree in Psychology and Nursing from Nazareth College of Rochester. Mr Parikh received his MS Degree (Psychiatric and mental health nurse practitioner) from Hunter College (City University of New York). Mr Parikh provided Nursing care and did his clinical rotations for NP school at one of the leading university hospital in the country, New york Presbyterian Hospital: The University hospital of Columbia and Cornell.
(1) What services/Treatment options are provided at Parikh Mental Health Services?
At Parikh mental health PLLC we provide psychiatric medication management, psychotherapy and other holistic treatment approaches for mental health conditions.
(2) Which medications do you prescribe and not prescribe?

We are unable to answer questions specific to which medication we prescribe or not as our clinicians make decisions based on your history, evaluation, diagnosis, and other factors and then determine appropriate treatment options for you. Please note that we follow all the federal, state specific and DEA guidelines on medications and control substances.
(3) Do I need a referral to be seen and establish care at Parikh mental health services?

At Parikh mental health PLLC we see our clients for establishing mental health treatment based on self referral or a referral from their primary care provider or any other clinician they may be seeing.

(4) How do I schedule my first and follow up appointments?
​
At Parikh mental health services, PLLC you can schedule an initial psychiatric evaluation by calling 704-255-5070 or email us at Info@Parikhmhs.com.
Once we receive an email with your information, you will be sent a link to create a patient portal.
Once you create the portal, Please ensure you upload a Valid government issued ID (driver's license, passport etc), Proof of your residency status in North carolina (such as copy of driver's license, and/or utility bill). Please ensure you provide a working debit/credit card as a form of payment to stay on your file.
Please sign the CONSENT forms. After this you will be able to schedule an APPOINTMENT with date and time of your choice as long as we have availability.
Once you have selected the date and time, please fillout necessary documents/forms such as PHQ9 , GAD7 etc in its entirety on the portal before your Scheduled Initial Psychiatric Evaluation.

(5) What to expect during my initial psychiatric evaluation appointment and follow up appointments?
During the initial psychiatric evaluation, your clinician may gather history about yourself in terms of past psychiatric and medical history, family psychiatric and medical history, your past treatment and responses to treatment such as past medication trials and responses/outcomes, past psychotherapy or substance dependency/abuse/addiction treatments and outcomes, or any other form of treatment you may have received for your mental health and wellbeing. Your clinician will discuss in detail about your current symptoms that you may be experiencing that may be affecting or impacting your daily life in personal, social, work, and educational settings . During the evaluation, your clinician may also ask you to provide detailed history about your childhood, adolescence, adulthood, etc that may be relevant to your health and mental health. Based on information available to clinician at the time of evaluation, your clinician will discuss your diagnosis and may recommend pharmacological intervention and/or non-pharmacological intervention such as psychotherapy.

(6) Am I required to have Blood work/labs before or during on going care?
​
Prior to, during, or after the evaluation, you may be required to do baseline labs/blood work, which may include, but not limited to CBC with differential, complete metabolic panel vitamin B 12 , vitamin D levels, any current medication specific levels such as valproic acid and/or level lithium levels but not limited to this.
You will be responsible to schedule the blood work at the laboratory of your choice and you will be responsible to provide payment for this services to the lab directly. Please note that your clinician may not initiate your medication treatment or pharmacological intervention until results for the lab work are received and reviewed. Labs and Blood work may be ordered on routine basis based on your individual treatment plan.

(7) What if I do not agree or do not want to try the treatment options provided by the clinician?
​
Please note that if you and your clinician have disagreement in terms of diagnosis and treatment plan and you decide not to initiate and follow the clinicians recommendation you have right to speak and be seen by another clinician at Parikh mental health services, if one is available. In the event if no other clinician is available, you can be referred to another service agency of your choice. Please note if disagreement took place during Initial psychiatric assessment/evaluation, you are not considered established patient at Parikh mental health PLLC and at the end of the evaluation/visit, you will be considered discharge from our care and you will be responsible for establishing care elsewhere. please note if you and your clinician agree on the Diagnosis and treatment plan , you will be given subsequent follow up appointment/s to discuss the effectiveness and response to your treatment. At any point during your treatment at Parikh mental health service, if you disagree or are not content with the Clinician's recommendation and treatment plan, you have right to choose another clinician if one is available at Parikh mental health service or you have right to be referred elsewhere or simply ask to be discharged from our care. In such instance it is extremely important that you establish care at other agency/provider ASAP.
(8) Do you have No Show/cancelation policy?
In order to continue receiving prescription for your treatment plan, you are expected and required to keep your appointments as per your clinician’s recommendation. In the situation if you are unable to keep your appointment, we require you notify us at least 24 hours in advance. In such instances we will make best attempt to give you a follow up appointment on the next available appointment basis and your clinician may provide you a bridge supply Prescription for your medication treatment. Please note that any no-shows may result in no prescriptions renewed until visit scheduled and you are seen. Please note 2 no show appointments may result in your discharge from our services.
(9) What to do during emergency/crisis situation or does Parikh mental health services you provide emergency/crisis services?

Please note we are NOT able to provide EMERGENT care during or in CRISIS situations. if you find yourself or your loved one in such situation, where you or your loved ones life is in danger, immediately call 911 or call the national suicide hotline or your state specific crisis intervention number. (10) Does Parikh mental health services except insurance or how do I pay for the services or when are the payments due?
Please note that at this time we DO NOT bill or are NOT in network with any commercial, private federal, state, Medicare and or Medicaid insurance. Though we are able to provide receipts and all the relevant information to you, which you can submit it to your health insurance to get reimbursed.
At this time, we are only cash Pay service and all the payments are due at the time of visit. Please note that you will be required to have a working payment option such as a credit card or a debit card on file. your credit card or debit card will be charged for the services you received at the end of the visit. If a payment method is declined or no longer working, you are responsible for accrued payments. Please note that if you are unable to pay in full, we may be able to accomodate split payments and/or a payment plan.

(11) What are the costs for services?

We have FEE FOR SERVICE option

Initial Psychiatric Evaluation (upto 60 minute per session) = $ 300
Psychiatric medication management follow up visits (upto 30 minutes per session) = $ 150
Psychiatric medication management with Therapy (upto 45 minutes per session) = $ 225
Psychotherapy/Therapy sessions (30 minutes = $ 60, 45 minutes = $ 90, 60 minutes = $120)
