Please review these Terms and conditions carefully, as accessing or using the services or paying for the Services, you accept and agree to be bound by these Terms and conditions and acknowledge the Privacy Policy and other policies listed. If you do not agree to be bound by these Terms and conditions, you are not authorized to access or otherwise use the Services and you must promptly exit and stop using the Services. In these Terms and conditions, the terms “you” and “your” mean you: as the user of the Services, your dependents if any, any other person accessing your user account, and/or each of your heirs, assigns, and successors.
Control Substance Policy
At Parikh Mental Health Services, we are dedicated to implementing evidence-based practices, bolstered by our clinicians' extensive expertise in managing mental health conditions. Our team consistently follows established federal, state, and DEA-specific protocols and guidelines when considering pharmacological treatment options. It is important to recognize that our clinicians may use stricter approach when prescribing control substances under specific circumstances. Control substances may not be prescribed: (1) when being considered as a first-line treatment option, (2) during patient's first visit, (3) in response to any alterations in federal, state, or DEA protocols, such as the necessity for in-person consultations, and (4) if a clinician feels uneasy about prescribing controlled substances for any unspecified reasons. We prioritize your safety and well-being, ensuring that our treatment methods remain both responsible and suitable.
At Parikh Mental Health Services, our primary focus is on ensuring the safety and well-being of all our clients. When a controlled substance is either prescribed or being as part of your treatment plan, clinicians may adopt several protocols to monitor your care effectively.
These protocols may include (1) Requiring urine drug screens or blood work before and after the prescription of a controlled substance to ensure proper usage. (2) Furthermore, ongoing urine drug screens or blood work may be required prior to the authorization of refills and could be conducted at any point during your care as long as a controlled substance continues to be part of your treatment. (3) our clinicians will review state and federal databases to gather information on previous controlled substance prescriptions that you may received from other prescribers, whether they were past or current. (4) Clients may be asked to perform pill counts and/or limit the quantity per Prescription. (5) clinicians might also request that clients adhere to additional measures tailored to their individual treatment plans, which may not be explicitly mentioned here. We appreciate your understanding and cooperation in following these guidelines to ensure the best possible outcomes for your health.
**Fees for services**
1) Initial Psychiatric Evaluation (upto 60 minute per session) = $ 180
2) Psychiatric medication management follow up visits (upto 30 minutes per session) = $ 100
3) Psychiatric medication management with Therapy (upto 45 minutes per session) = $ 150
4) Psychotherapy/Therapy sessions (30 minutes = $ 60, 45 minutes = $ 90, 60 minutes = $120)
**New Client Packages**
From time to time Parikh Mental Health Services, PLLC may offer "New Client Packages" that may offer discounts on our services. You may receive these offers in mail or email as part of our marketing campaign. Please Note that if you have received such offer via mail or email, you should confirm with our staff of its active status as these offers are subject to change or canceled without any notice. By Default, any such offer is ONLY valid for 3 months or 90 days from the day mailed or emailed or Posted on our website or any other sites we post on.
**Bundled Visit Packages**
We are pleased to offer our "Bundled Visit Packages", meticulously designed to provide comprehensive care while considering affordability. These Packages are ideal for our established Clients, but they are also available for our New Clients as well. Our Bundled Visit Packages are designed to provide comprehensive care at a discounted rate. These packages combine an initial psychiatric evaluation with follow-up medication management visits, or offer multiple medication management follow-ups in one convenient Payment Plan. Explore our various options to find the perfect fit for your mental health journey while saving on costs. Your well-being is our priority, and we’re here to support you in the best way we can.
Below are the details of each available "Bundled Visit Package options:
**Option 1:** This package includes ONE Initial Psychiatric Evaluation Visit, followed by TWO Medication Management Follow-Up Visits, all for a total cost of $300. The standard price for these services would amount to $380, allowing you to save $80 by choosing this bundled package.
**Option 2:** This option features ONE Initial Psychiatric Evaluation Visit in conjunction with FIVE Medication Management Follow-Up Visits for a total of $600. The regular price for this combination of services is $680, resulting in a significant savings of $80 when you select this bundled offer
Any Additional visits after full utilization of visits from the subscribed Bundled Package options, will be charged at the rates mentioned in the "Fees for Services" section
We invite you to take advantage of these thoughtfully curated and cost-effective packages, which are designed to meet your healthcare needs ensuring you receive the highest quality of care at a reduced cost.
We want to ensure you understand our Bundled Visit Packages. All Options are valid for ONE year from the date of purchase, with full payment required at the time of subscription. Please note that once you begin the use of your selected package with your initial evaluation or a follow up visit, the package is non Cancelable and non-refundable. If, for any reason, you have to be discharged from our care prior to all visits are utilized, your final payment will be based on the number of visits used, in accordance with our "fees for services" schedule, but will not exceed the total cost of "Bundled Visits Package" you selected.
(Example 1: if Option 1 is canceled after 2nd visit and before the 3rd visit, Total cost will be determined per "Fee for Services": 1 Initial Psychiatric Evaluation ($180) + 1 Medication Management Follow up visit ($100) = which equals to total cost of $250. In this scenario Parikh Mental Health Services will Issue a refund of $20 to Client).
(Example 2: If Option 2 is canceled after the 5 th Visit then cost will be determined as per "Fees for services" : 1 Psychiatric Evaluation ($180) + 4 Medication Management follow up visit (each at $ 100 equals to $ 400) = $ 580. In this scenario Parikh Mental Health Services will Issue a refund of $20 to Client).
Please Note that None of the currently Offered Discounted Packages ( such as "Fees For Services", "Bundled Visit Packages" and/or "New Client Packages" can be combined).
Please note, None of the offered Packages are an offer of Health Insurance coverage and Parikh Mental Health Services DO NOT provide Health Insurance coverage of any type.
**Cancellation Policy**
If you are unable to keep your appointment, we ask that you notify us at least 24 hours in advance. In such cases, we will make our best effort to reschedule you at the next available appointment. Cancellations made less than 24 hours before your scheduled time may incur a fee of $35 as part of our late cancellation policy. For example, if your appointment is set for 10 AM on 12/02/0000 and you cancel at 10:30 AM on 12/01/0000, this be considered a late cancellation.
**No Show Policy**
For established patients, please note that any no-shows may result in no prescriptions being renewed until you have scheduled and attended a visit. The provider may offer a bridge supply of medication if deemed appropriate. Each no-show will result in a $75 fee charged to your credit or debit card at the end of your scheduled time slot. Additionally, two consecutive no-show appointments may lead to your discharge from our services.
